AH-1G 67-15831 was conducting a reconnaissance mission for armed escort in South Vietnam. While in level flight at 500 feet and 100 knots, the helicopter was struck by small arms fire (7.62mm), hitting the tail section and damaging the tail rotor. As a result, a forced landing was made, and the aircraft was recovered without using its own power. Both the mission and the helicopters flight capability were terminated.

Despite the damage, the helicopter continued flight and successfully completed all mission objectives.
