    Constructed as a Norseman V.

    19 September 1946

    To Saskatchewan Government Air Ambulance Service, Regina, SK with new c/r CF-SAM.
    The second plane to fly for the Saskatchewan Air Ambulance Service, North Americas first civilian air ambulance program.

    14 June 1950

    To Saskatchewan Government Airways, Prince Albert, SK keeping c/r CF-SAM.
    Modified with a belly exit to allow the dropping of the Saskatchewan Smoke Jumpers when on floats.

    1962

    Involved in an incident.
    Swept over a set of rapids above Nistowiak Falls, Saskatchewan. The wings were removed, and a road cut through bush, in order to move the aircraft back to safe water.

    To Saskair, Prince Albert, SK keeping c/r CF-SAM.

    3 September 1965

    To Norcanair, Prince Albert, SK keeping c/r CF-SAM.

    17 December 1965

    To Dolphin Airways, Lynn Lake, MB keeping c/r CF-SAM.

    4 January 1967

    To La Ronge Aviation, La Ronge, SK keeping c/r CF-SAM.

    20 December 1967

    To Nipawin Air Service, Nipawin, SK keeping c/r CF-SAM.

    15 July 1976

    Civil registration, CF-SAM, cancelled.

    To Saskatchewan Tourist Association and Western Development Museum.

    Restored, and painted in the original Air Ambulance gold and green colors.

    To Saskatchewan Western Development Museum, Moose Jaw, SK.
