    Constructed as a CF-104D Mk 2.

    From 3 November 1964 to 28 July 1970

    Taken on Strength/Charge with the Canadian Armed Forces with s/n 12666.

    Taken on Strength/Charge with the Canadian Armed Forces with s/n 104666.

    Transferred to No. 441 Squadron, 1 Canadian Air Group, Europe.
    Not popular with maintenance crews, known as Triple Pig and the Jet from Hell. Only flew 50 hours in its last four years in service.

    5 March 1975

    Crashed.
    Destroyed in takeoff abort on 5 March 1975, at CFB Baden-Sollingen, Germany. Was number four in a flight to Suippes Range in France. Main tire blew at 190 knots, aircraft reached 215 knots on the ground before abort was started. Aircraft missed barrier, and shed pylon tanks and fuel starting a fire. Both crew exited aircraft after it came to a stop, without injuries.

    During the crash the canopy did not jettison properly. Subsequent investigations by AETE led to canopy modifications on all duals.

    Struck off Strength/Charge from the Canadian Armed Forces.

    June 1994

    Parts of airframe noted on Cold Lake fire training dump.

    Remainder of airframe may have been used for Aircraft Battle Damage Repair training at Cold Lake, but this is not confirmed.

    January 2010

    To Cold Lake Museum.
    Only the nose survives here.
