    Mid-air collision with F-106B 57-2521, piloted by Col John Anderson. Lost half right wing. Returned, landed safely.

    Transferred to the 309th Aerospace Maintenance and Regeneration Center (AMARC) with inventory number FN168.

    12 September 1992

    Converted to a QF-106A.
    AEL Inc., East Alton IL for drone conversion.

    16 December 1992

    Taken on Strength/Charge with the United States Air Force with s/n AD214.

    2 June 1993

    Shot down.
    Shot down on 1st NULLO by an AIM-7M.
