    Leased to Bell Helicopter, Fort Worth, TX.
    Bailment.

    From November 1966 to February 1967

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    March 1967

    Transferred to Post Supply, 6th Army, Fort Ord, CA.

    From April 1967 to May 1967

    Transferred to Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    From June 1967 to July 1967

    Transferred to U. S. Army Republic of Vietnam, Flight Detachment, Vietnam.

    Damaged.
    This was a Combat incident. This helicopter was REPAIRED IN THEATER. This helicopter was on Take-Off at 0200 feet and 060 knots. Helicopter took 2 hits from:Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (7.62MM). The helicopter was hit in the Cockpit. Systems damaged were: PERSONNEL. Casualties = YES. The helicopter continued and accomplished all mission objectives.

    Aircraft bailed to Bell Helicopter while stationed in Vietnam and operated by them in Vietnam.

    Damaged.
    This was a Combat incident. This helicopter was REPAIRED IN THEATER. This was a Recon mission for Unarmed Recon. While Enroute this helicopter was Unknown at 1000 feet and 080 knots. Helicopter took 1 hits from: Small Arms/Automatic Weapons; Gun launched non-explosive ballistic projectiles less than 20 mm in size. (7.62MM). The helicopter was hit in the Main Rotor Blade System. Systems damaged were: MAIN ROTOR SYSTEM. The aircraft continued and accomplished all mission objectives.

    Converted to an UH-1H.

    Transferred to HQ and HQ Detachment, 44th Engineer Group, Thailand.
