    Constructed as a F-4B-15-MC by McDonnell at Saint Louis, Missouri, USA.

    First flight.

    May 1963

    Taken on Strength/Charge with the United States Navy with BuNo 150994.
    Although built as an F-4B, this jet was sent directly to the USAF for F-4 testing and early training before F-4Cs were delivered.

    1 May 1963

    Taken on Strength/Charge with the United States Air Force with s/n 62-12180.
    Loaned to the USAF for testing.

    7 May 1963

    To 4453rd CCTS, 4453rd Combat Crew Training Wing, MacDill AFB, FL.
    Markings added: FJ-180

    June 1963

    Taken on Strength/Charge with the United States Navy with BuNo 150994.

    Transferred to VF-92.
    Operated with markings: NG-___

    7 June 1968

    Destroyed in a crash.
    Just after takeoff from the USS Enterprise, the aircraft suffered control column lock. The crew successfully ejected before the jet crashed into the Gulf of Tonkin off of North Vietnam.
