    Constructed as an UH-1C-BF by Bell at Fort Worth, Texas, USA.

    Circa March 1967

    Taken on Strength/Charge with the United States Army with s/n 66-15062.

    March 1967

    To Office of Director of Con. Supply, 1st Army, Fort Knox, KY.

    April 1967

    Transferred to A Troop, 7-17 Air CAV., 1st Army, Fort Knox, KY.

    September 1967

    Transferred to 7-17 Air CAV., 1st Army, Fort Knox, KY.

    November 1967

    Base of operations changed.
    Sent to South Vietnam for combat operations with 7-17 Air CAV.

    22 January 1968

    Damaged.
    While descending at 1,050 feet and 100 knots en route to the mission in South Vietnam, UH-1C 66-15062 was struck once by 7.62mm small arms fire. The hit impacted the tail section, but the extent of system damage is unknown. Despite the hit, the helicopter continued flight and successfully completed all mission objectives.

    Transferred to HQ Company, 7-17 Air CAV., South Vietnam.

    27 April 1968

    Damaged.
    During an attack run at 500 feet and 95 knots in the target area in South Vietnam, UH-1C 66-15062 was struck once by enemy 7.62mm small arms fire. The round hit the armament system, which sustained damage. Despite this, the helicopter continued flying and successfully completed all mission objectives.

    August 1968

    Transferred to 134th Assault Helicopter Company, Phu Hiep, South Vietnam.

    6 December 1968

    Damaged.
    While UH-1C 66-15062 was hovering, the engine failed, prompting an autorotation and hard landing. A fire occurred in the engine compartment but was extinguished. The engine failure was caused by foreign object damage (FOD), identified as green tape.

    August 1969

    Transferred to 79th Transportation Company, South Vietnam.
