    Constructed as a CH-46A.

    15 October 1965

    Taken on Strength/Charge with the United States Marine Corps with BuNo 151952.

    21 December 1965

    Transferred to Marine Medium Helicopter Squadron 164 (HMM-164), MCAS El Toro, CA.
    Operated with markings: YT

    From 1966 to March 1967

    Deployed to Vietnam.

    23 March 1967

    Transferred to HMM-265, MCAF Jacksonville, NC.

    July 1967

    Deployed to Vietnam conducting heliborne search and destroy missions against Viet Cong targets along the coast.

    November 1967

    Carried Raquel Welch and Bob Hope during USO tour.

    Damaged.
    Shortly after carrying Welch and Hope the aircraft was shot down and cannibalized.

    By January 1968

    Repaired and returned to flight status.

    27 July 1968

    Damaged.
    Sand clogged the fuel filter on engine number 1, and aircraft lost power lifting an external load of 105 rounds at hill 55, and crashed into the ammo dump. She was lifted to MMAF where HAMS and a Boeing Vertol tech repaired her.(Statement by former crew chief).

    Transferred to HMM-165, MCAS Santa Ana, CA,.
    Deployed to Vietnam.

    25 October 1969

    Transferred to HMM-164, MCAS El Toro, CA.
    Deployed to Okinawa.

    27 April 1970

    Transferred to HMM-165, MCAS Santa Ana, CA.
    Deployed to Vietnam.

    5 September 1970

    Transferred to Headquarters and Maintenance Squadron 36 (H and MS-36), Okinawa.

    2 February 1971

    Transferred to Marine Air Reserve Training Detachment, NAS Whidbey Island, WA.

    21 January 1972

    Transferred to the Military Aircraft Storage and Disposal Center (MASDC) with inventory number 1J0037.

    21 May 1975

    Taken on Strength/Charge with the United States Navy with BuNo 151952.

    21 May 1975

    Transferred to Naval Air Rework Facility, NAS North Island, CA.

    Ferry flight. Delivered to National Naval Aviation Museum, NAS Pensacola, Pensacola, FL.
