    Summary: The airframe was written off. There were four fatalities. There were 17,585 hours on the airframe at the time. The ambulance flight departed St. Anthony Airport, NF (YAY/CYAY), Canada and was destined for Goose Bay Airport, NF (YYR/CYYR), Canada. The accident occurred at Mealy Mountain, NF (Canada) while on approach. Narrative: The Twin Otter departed Goose Bay on a medical evacuation flight to St. Anthony.
