    Constructed as an UH-1D-BF by Bell at Fort Worth, Texas, USA.

    Circa 1966

    Taken on Strength/Charge with the United States Air Force with s/n 66-16019.

    April 1967

    To Army Aviation Division, 6th Army, Dugway Proving Ground, UT.

    June 1967

    Transferred to U. S. Army-Vietnam Flight Detachment, South Vietnam.

    August 1967

    Loaned to Bell Helicopter Company, Fort Worth, TX.

    September 1967

    Transferred to 7-17 CAV., 1st Army, Fort Knox, KY.

    November 1967

    Transferred to B Troop, 7-17 CAV., 1st Army, South Vietnam.

    16 February 1968

    Damaged.
    During a Close Air Support combat mission, UH-1D 66-16019 was flying at 1,000 feet and 80 knots in the target area when it took three hits from 7.62mm small arms fire on the right side. The extent of the damage is unknown, but the helicopter remained operational, continued its flight, and successfully completed all mission objectives. It was later repaired in-theater.

    May 1970

    Transferred to 604th Transportation Company, South Vietnam.

    Returned to the United States.

    September 1970

    Transferred to Office of Naval Research, University of Michigan, NAS Pensacola, FL.

    January 1971

    Transferred to Naval Air Test Center (NATC), NAS Patuxent River, MD.

    March 1971

    Transferred to 52nd Artillery Brigade, 1st Army, Fort Hancock, NJ.

    February 1973

    Transferred to 44th Aviation Det., 1st Army, Stewart AFB, NY.

    March 1973

    Transferred to 52nd Artillery Brigade, 1st Army, Fort Hancock, NJ.

    May 1973

    Transferred to HHB, 45th Field Artillery Brigade, Fort Sheridan, IL.

    July 1973

    Transferred to 16th Air Defense Artillery (ADA) Group, 1st Army.

    September 1974

    Transferred to the Military Aircraft Storage and Disposal Center (MASDC).

    Permanently withdrawn from use.
    Log books show the Huey had more than 21,000 hours of flight time throughout its life.
