    Involved in an incident.
    Forced down at sea near Adak, Alaska, with the failure of the port engine. The situation was worsened by the accidental feathering of the starboard engine requiring major overhaul of the airframe and engines.

    There was one fatality and were two minor injuries. The accident occurred at the Lewistown Airport (LWT) Lewistown, MT, United States while standing. The damage was substantial. During taxi to takeoff the brakes failed. The pilot attempted to groundloop the aircraft. The aircraft rolled down hill and collided with a gully. The pilot was fatally injured. During preflight the co-pilot inadvertently added hydraulic fluid in the alcohol tank. The hydraulic pump cavitated...
