    Constructed as an UH-1H-BF by Bell at Fort Worth, Texas, USA.

    Taken on Strength/Charge with the United States Army with s/n 67-17250.

    To 155th Assault Helicopter Company, South Vietnam.

    Damaged.
    UH-1H 67-17250 departed Dong Ba Thin for Ban Me Thout at approximately 1530 hours on the return flight of an admin run. The aircraft was loaded with four passengers, the baggage of two passengers, two small refrigerators and it had 1300 pounds of fuel on board. A hover check was made prior to take-off with no loss of RPM. The gauges cannot be recalled but they were such that the aircraft commander let the pilot make the take-off (indicating torque and N1 were not near critical take-off conditions). At approximately 1600 hours on a preferred flight route northwest of Ninh Hoa at 3500 feet indicated the master caution light came on. It indicated spare. The aircraft commander took the controls when panel lit. Ten to fifteen seconds later the engine failed. The aircraft commander entered autorotation noticing 0 engine RPM and 0 torque. The aircraft commander continued the autorotation, attempting to reach a nearby road; however, the aircraft fell short of the road approximately 5 to 10 feet. In the final flare prior to touch down main rotor blades struck a tree. The RPM was not sufficient to insure a soft touch down. Upon landing, the main rotor blade struck a stump. The may day call was answered by an aircraft in the Nha Trang area. A perimeter was set up around the aircraft. They were on the ground 45 minutes to an hour and 15 minutes before the crew and the aircraft were picked up and returned to Ban Me Thout. Within two and a half hours after the accident all personnel were examined and released by the Flight Surgeon.

    Transferred to 62nd Aviation Company, South Vietnam.

    January 1972

    Transferred to 173rd Assault Helicopter Company, South Vietnam.

    March 1972

    Returned to the United States.

    July 1972

    To ARADMAC, NAS Corpus Christi, TX.

    August 1972

    Transferred to Director of Maintenance, 3rd Army, Atlanta Army Depot, GA.

    July 1973

    To 281st Aviation Compnay, 5th Army.

    November 1975

    Transferred to Director of Supply, Corpus Christi Army Depot, TX.

    By 1996

    Converted to an UH-1V.

    By 1996

    Transferred to D Company, 109th Aviation Battalion, Iowa Army National Guard, Boone, IA.

    By September 2000

    To Army National Guard, Boone Municipal, Boone, IA.
    Mounted on a plinth near the airport entrance adjacent to the ARNG building.
